Consent & Authorization

By signing below, I voluntarily grant permission to Mothers' Milk Bank Southeast and its representatives, employees, partners, and affiliates to use my submitted story, quotes, photographs, video, and/or likeness for educational, promotional, fundraising, advocacy, and marketing purposes.

I understand and agree that:

• My story and/or images may be used in print and digital formats, including but not limited to newsletters, brochures, annual reports, websites, email communications, social media, presentations, press materials, and other publications.

• My story may be edited for spelling, grammar, punctuation, length, formatting, or clarity while preserving the original meaning and intent.

• Portions of my story may be condensed or excerpted for publication purposes.

• My first name and relationship to donor milk may be used unless I request otherwise below.

• I understand that submission of my story and photos is voluntary and that I will not receive financial compensation for their use.

• I certify that any photos submitted are mine to share or that I have permission from the photographer and any individuals pictured.

• I understand that once materials are published online or shared publicly, Mothers' Milk Bank Southeast may not be able to fully prevent further sharing by others.
